"Sir, aapko 4500 grafts lagenge." This is the most common opening line in any clinic in India — said within thirty seconds of looking at your scalp, often without a single density measurement. The number is usually wrong. Here is how to estimate honestly, what each Norwood stage actually needs, and how to spot inflation before you pay for it.
What is a graft (and why the definition matters)
A graft is a follicular unit — a natural cluster of 1, 2, 3, or sometimes 4 hairs that grow from the same pore. The average Indian scalp has roughly 2.0–2.2 hairs per graft. So 2000 grafts typically delivers about 4200–4400 actual hairs.
This matters because some clinics quote "hairs" while others quote "grafts" — and use it interchangeably to confuse you. If a clinic tells you "5000", ask whether that is grafts or hairs. The 2x–3x difference is where most overcharging hides.
The Norwood scale — find yours first
Before any graft number makes sense, you need to know your Norwood stage. Google "Norwood scale chart" and match your hairline honestly — not the version of you from 3 years ago. The seven main stages, in plain language:
- NW1: No real loss. You don't need a transplant.
- NW2: Mild temple recession, "mature hairline".
- NW3: Clear temple thinning with M-shape. NW3V adds vertex thinning.
- NW4: Front + crown thinning, but with a band of hair separating them.
- NW5: Front and crown almost connecting, band weakening.
- NW6: Front and crown fully connected — large bald area on top, U-shape donor.
- NW7: Only a narrow horseshoe of hair left around the sides and back.
Norwood 2 — 800 to 1500 grafts
Most NW2 patients don't need a transplant at all. A 5-month course of minoxidil + finasteride often halts the progression. If you still want corner reinforcement, 800–1500 grafts is enough — placed only in the receded triangles.
Red flag: Any clinic quoting more than 2000 grafts for a NW2 is selling you density, not restoration. You will look unnatural and waste donor for nothing.
Norwood 3 / 3V — 1800 to 2500 grafts
This is the most common bracket. Hairline + corners typically need 1500–2000 grafts. If there is also vertex (3V) involvement, add another 400–600.
Density target should be 35–45 follicular units per cm² in the frontal zone for a soft, natural look. Many Indian clinics under-plan at 25 FU/cm² to save grafts — the result is a transparent hairline that's visible by month 12.
Norwood 4 — 2500 to 3200 grafts
Front + corners + the thinning band behind. Now donor planning starts to matter — you don't want to over-extract in session one and have nothing left if you progress to NW5 in 5–7 years.
A responsible surgeon will discuss long-term medication (finasteride) to lock the existing native hair. Without medication, the native hair behind your transplant will keep falling and you will return for a touch-up that may not be doable.
Norwood 5 — 3200 to 4200 grafts
This is where one-session full coverage becomes hard. Either you accept a thinner overall density to cover everything, or you focus on the front 2/3 first and leave the crown for a second session 12 months later.
Most experienced surgeons will recommend the second approach. Crown is a low-priority zone — invest the donor where it frames the face first.
Norwood 6 — 4200 to 5500 grafts (two sessions)
NW6 means a large bald area on top. Realistically, this is a two-procedure plan. Session 1 builds the frontal 2/3 with around 3500 grafts. Session 2, after 10–14 months, adds 1500–2000 grafts for crown filling.
Anyone promising NW6 coverage in a single 4000-graft session is either ultra-low density (looks bald in photos again at month 12) or counting hairs instead of grafts.
Norwood 7 — usually not advisable
NW7 patients have very limited donor — often only 4500–5000 safe grafts left. Covering the entire scalp would require 6500+, which exceeds the donor budget. Most experienced surgeons will either decline NW7, or do a "framing" session of 3000–3500 grafts to recreate just the front zone for facial framing — leaving the crown bald and shaved short.
Beware any clinic that says "yes" to a full NW7 transplant. It almost always ends in donor scarring + an unnatural looking final result.
Donor maths — your lifetime budget
The donor zone (back and sides of your scalp) is finite. Most patients have:
- Safe lifetime donor: 5500–7000 grafts (Indian average closer to 5500–6000)
- Single-session safe extraction: Up to ~3500–4000 grafts in one day
- Healing window between sessions: 10–14 months minimum
Over-extracting in session one — say, 4500 grafts done aggressively — can permanently visible-thin the donor, leaving "see-through" patches that show when you shave. This is irreversible. Once the donor is damaged, no procedure restores it.
How clinics inflate the number
Common tactics I've heard from clients over the past 4 years:
- Hair count instead of graft count. "5000" can actually mean 2200 grafts × 2.3 hairs.
- Splitting multi-hair units. A 3-hair graft is split into three "single-hair grafts" to triple the number.
- Counting test extractions. The 50 grafts pulled to check donor quality get added to the final invoice.
- Per-graft pricing with no upper cap. A 2800-graft procedure gets "expanded" mid-surgery to 3600 because "the area needed more". Always lock a fixed price.
- Inflated Norwood diagnosis. You're NW3 but they call you NW5 to justify 4500 grafts.
